These documents aim to aid the nine Commissioning Localities (shown on the map above) in the identification of health needs, priority areas and potential service interventions that could be commissioned or provided. The information, together with other data held within the Commissioning Locality, can then be used as a rudimentary health needs assessment and could be used to inform decisions within the locality.
The 2016 Profiles were published in December 2016 and can be downloaded from the links below. Previous editions (known as GP Federation Profiles prior to 2015) were published in 2015, 2014, 2013 and 2012, and are also provided through the tabs below.
